How To Enter Paper Surveys

Have a survey that you are running?  Have certain employers that prefer paper surveys? Not a problem, just make sure that your paper survey has all of the matching fields as your online survey and input those surveys into the online system via some good old fashioned manual data-entry! With the paper surveys in hand, you can quickly and easily submit that data via your survey URL and have all of the information in one place and on one export, so that you can easily do your calculations.

Below is an example of a survey that was done on our survey platform and the instructions that were given to the data-entry consultant:

Each page in the PDF accounts for one entry on the online survey.  Below you will see specific directions for each question, and where you can find the information on the PDF form.  The first picture will be the area where you will enter it online, the second picture is where you can find that information on the PDF page. If there are any fields missing on the PDF page, enter it as ‘not entered’ unless specifically mentioned in the step.

1. Go to the survey’s online URL (ex. https://yoursystem.com/s/your-survey-name)
2. Enter the person’s first name.

4. Enter the person’s work email.  If there is no email listed (or they put N/A), put none@none.com as the entry.

5. Enter the person’s work phone number.

6. Enter the person’s home zip code.

7. Enter the person’s mileage.  If there is not a number entered on the PDF page, enter it as 1.

8. Enter the person’s company.  This is a dropdown that you will be able to find the correct company. The data for this is in the top-left hand corner of the PDF page.

9. You will now go through the days of the week for the arrival modes.  The users have put in the corresponding letter of their mode, which you will be able to find in the drop-down menu of the online survey.  If they chose a mode that is not a carpool or vanpool, you will only have the mode and time to fill in.  If they chose carpool or vanpool, you will need to fill in the number of riders and check the box if they put ‘Y’ for if the driver works at HHC. Do this for Monday through Friday in the arrival section.

10. You will now go through the days of the week for the departure modes.  This is essentially the same as entering the arrival modes. The users have put in the corresponding letter of their mode, which you will be able to find in the drop-down menu of the online survey.  If they chose a mode that is not a carpool or vanpool, you will only have the mode and time to fill in.  If they chose carpool or vanpool, you will need to fill in the number of riders and check the box if they put ‘Y’ for if the driver works at HHC. 11. If any fields have been missed, or incorrectly filled out, you will see this warning.

12. If everything is filled out completely, you will see this message.

13. Click the ‘Send Response’ button and it will submit the survey and give you a message stating that everything went through.
14. Repeat for the next page of the PDF
